Individuals under the age of 18 years old must come in with a parent or guardian.
Upon application, borrower’s cards will be issued without charge to any resident living within the village limits of Midlothian.
VERIFICATION OF ADDRESS MUST BE FURNISHED.
Illinois residents who live outside the boundaries of the Midlothian Public Library and are not served by any other public library, may purchase a nonresident card for an annual fee of $185.
NONRESIDENT TAXPAYER CARD
Individuals residing beyond the jurisdictional boundaries of the Midlothian Public Library but owning taxable property within these boundaries may obtain one (1) individual non-resident library card without payment of the non-resident fee upon presentation of the most recent tax bill.
NONRESIDENT BUSINESS CARD
Cards may be issued to business owners who are non-property owners for a period of one year to be renewed annually as long as the business is in Midlothian. The business owner must show proof of ownership with an official piece of identification (i.e., rental agreement, utility bill, official business letter or bill, etc.), showing his/her name and name of the business to obtain the card. The card is issued in the name of the owner to be used by the owner.
a. All patrons must fill out an application card. Identification, proof of address and telephone number are required for new applications. Items accepted as proof of address are driver’s license, state ID card, or a dated piece of first class mail. The application card for children under 18 must be signed by a parent or legal guardian. A child must be able to print his/her name legibly on the application in order to receive a card.
b. Non-residents must complete an application card, and must pay the $95 fee at the time the application is accepted. Non-resident cards are issued for one year.
c. A $3.00 fee will be charged to replace a lost card that has not expired. Once a card has been replaced, the original card is invalid. There is free replacement for a damaged card, when the damaged card is presented, once during a three-year period.
e. No replacement card will be issued to a delinquent borrower until his/her record has been cleared.
f. Verification must be furnished when a change of name or address is requested.
g. The Midlothian Library will not issue cards or provide service to patrons who are known to have overdue obligations (in the form of unpaid fines or overdue/lost/damaged material) at another library.
h. Borrower cards for patrons living within the limits of Midlothian are issued for three years.
